Outbreaks and disease surveillance
One monsoon has brought three outbreaks: Chandipura virus in Gujarat and Rajasthan, leptospirosis in Kerala, and an H1N1 surge in several cities. Each tests the same machinery. India's answer rests on guidelines of 2008, a national plan of 2019 and a laboratory network run by the Indian Council of Medical Research (ICMR), with testing that goes on all year but sets no alert threshold to act on.

Chandipura virus
- A virus, spread mainly by sandflies
- Fever that can progress to acute encephalitis syndrome, brain inflammation
- Mostly in children
Leptospirosis
- A bacterium, Leptospira; a zoonosis, not a person to person infection
- From water or soil contaminated by infected animals, through cuts or mucous membranes
- Farmers, cleaners and construction workers most exposed
Influenza A (H1N1)
- A virus, a subtype of influenza A
- Named for two surface proteins, haemagglutinin (H) and neuraminidase (N)
- Caused the 2009 pandemic, now circulates as a seasonal strain
- Heavy monsoon rain, floods and humidity help Leptospira survive, and rodents living near homes are its reservoir.
How surveillance works, and where it falls short
Testing
- Influenza like illness and severe acute respiratory infection (SARI) samples tested year round
- No alert thresholds
Coordination
- Split between ICMR, the National Centre for Disease Control (NCDC) and the States
Treatment
- Under 10 per cent of admitted patients get antivirals
- Surges are compared fairly by incidence rather than by raw case counts.
- Wastewater surveillance can give early warning of a novel pathogen.
Who does what
- The Ministry of Health and Family Welfare is the nodal ministry for a biological disaster.
- The National Disaster Management Authority (NDMA) issued Guidelines on Management of Biological Disasters in 2008, and the National Disaster Management Plan of 2019 sits alongside them.
- ICMR runs over 150 Virus Research and Diagnostic Laboratories and two mobile Biosafety Level 3 (BSL-3) laboratories.
- National Disaster Response Force (NDRF) teams are trained for chemical, biological, radiological and nuclear (CBRN) response.
- PM-ABHIM, the Pradhan Mantri Ayushman Bharat Health Infrastructure Mission, builds capacity against emerging diseases.
One Health and spillover
- Crowded, antibiotic dosed livestock farming drives antimicrobial resistance and the spillover of pathogens to people, which is what One Health is a response to.
What has happened
3 Sep 2026
Seasonal influenza surged across India, with H1N1 reported from several cities, making the surge a live test of the surveillance built after COVID-19.
- This season's vaccine matched the circulating H1N1 strain, but uptake is low.
- An integrated command structure was proposed to replace that split.

The Health Ministry set out how India prepares for a biological disaster, and what it listed was the pieces already in place since before the pandemic rather than one new plan.

At least 22 children had died of Chandipura virus in Gujarat and Rajasthan by 6 August.
- Mosquitoes, ticks and mites are being tested alongside the sandfly as vectors.
- Cattle, buffaloes and goats are being tested as reservoir hosts, so NCDC, ICMR and the Department of Animal Husbandry went in as one team.

Leptospirosis killed 50 people in Kerala in July alone, which made it the State's deadliest communicable disease.
- Late diagnosis is what carries the infection on to pulmonary haemorrhage and multi-organ failure.
